issues list updated

QAWG --

There is a new version of the Issues List at [1].

We (a task team of LR, DHM, LH, with comments from OT & KD) have gone 
though several issues whose discussion and/or statement of resolution was 
unclear, incomplete, or contradictory.  Plus a couple of cases of no 
documented discussion/resolution.  We have clarified the discussions and/or 
resolutions.  Pay particular attention to these issues:

98, 95, 91, 81, 78, 64, 39, 93, 92, 83

If you disagree with any of the clarifications, then please email to the 
QAWG list -- identify the issue and state the problem.    We do not intend 
to further discuss these individually (at least not the "Closed" ones), 
unless you raise a problem now.
