issues list updated

QAWG --

This version of the Issues List [1] is the most complete that I can do for 
now.  Below is a summary of current dispositions, as they have changed 
since pre-Tokyo.

In the first category below, there were problems with the resolutions, such 
that I could not get a write a clear and complete resolution -- documents 
were either incomplete or contradictory.  For some in this category, we 
should give some attention to Olivier's minutes before they are circulated 
as final.

The second bunch had no record of discussion or resolution.

I have left all in both categories in an "Active" state for now, to ensure 
that we sort them out, except for #83 where I clearly remember discussion 
(which was not minuted).

The attached text file describes the problem cases.
