[MathML3-last-call] mscarry should inherit its values, not have defaults

This was missed from being officially logged:

mscarry currently defaults the location and crossout values.  One part of
the spec says that the children of mscarries should implicitly be wrapped
with mscarry.  Because mscarry defaults its values, that means that the
crossout and location on mscarries would be ignored.  The fix is to make
mscarry inherit those values instead.

In addition to changing the values in the table, some of the examples that
use mscarry need changing.
