Linktypes Start and Index

Hello,

Can anyone clarify for me the difference between the link types Start and
Index? Is Index an index in the sense "starting page" (files named
"index.html") or an index in the sense "list of terms with links to their
definitions in the text"?

Also, are link types always used for external links, and not internal?
