Is the 'x' in &#xH; case sensitive? I presume the hexadecimal characters, themselves, are not. Although it's best to include the semi-colon terminator, is any !isxdigit((unsigned char)ch) an implied terminator, just as any !isdigit((unsigned char)ch) is an implied terminator for decimal character reference?
