[for archival] final set of comments on Eval Videos set 2

The attached document is a compilation of the EOWG comments from the 
survey of 4-11 December and teleconference call on 13 December, and 
confirmation survey of 20 December - 7 January and teleconference call 
on 10 January. This is the final set of comments for videos 1-4 (more 
work needs to be done of video 5).
