Karen, please accept my apologies for our oversight. You are completely correct that the platform we used to share our survey is not fully accessible. We have updated the form's introduction to inform those interested that we will do our best to share with them the survey in a different format. Can I share with you a text only version of the survey? Or if you can suggest another preferred format we will try to accomodate your suggestion.
