It's in Tim Berners-Lee's 1993 HTML 1.2 specification, that I referenced up thread, so at least 25 years old. I think it was in HTML 1.0, but I couldn't find that quickly. (Incidentally, to me the HTML 5.2 definitions look like attempts to give semantic legitimacy to B and I after the fact.)
