Survey Format suggestions, Adding Acceptance Criteria for SCs in Survey

I'd like to propose an addition to all surveys for Success Criteria. I
think each new SC proposal could have a yes/no radio question for each of
the Acceptance Criteria for SCs followed by an additional comments text
field


   1. Does it address a situation where a user with a disability will be
   disproportionately disadvantaged (as compared to a user without a
   disability) if the criteria is not met?  (Yes/No Radio)
   2. Is it be testable through automated or manual processes. (Yes/No
   Radio)
   3. Does it describe the specific condition required to meet the
   criteria, not the method to address the criteria. (Yes/No Radio)
   4. Does it utilize the WCAG 2.0 A/AA/AAA level structure. (Yes/No Radio)
   5. Does it ensure for revised Success Criteria that pages that meet the
   revised guidance continue to meet the corresponding WCAG 2.0 Success
   Criteria. (Yes/No Radio)
   6. Does it apply to all content unless preconditions for the application
   of the success criteria are explicitly identified (e.g. "except
   interruptions involving an emergency"). (Yes/No Radio)
   7. Does it apply across technologies to the greatest extent possible.
   (Technology-specific issues should usually be addressed in Techniques.)
   (Yes/No Radio)
   8. Does it avoid creating a requirement for something that is already
   required by an existing Success Criterion. (Yes/No Radio)
   9. Does it have Success Techniques which demonstrate that each Success
   Criterion is implementable, using readily-available formats, user agents,
   and assistive technologies. (Yes/No Radio)
