Survey for Re-chartering milestones

This survey uses a different form than the typical survey -- there are 
no options to check, just text boxes for open responses.

The survey asks you to review the text sections current charter and note 
if you see any changes that need to be made. (hopefully, none.)

The survey than asks you to predict how long the next steps will take. 
That has been broken down by the major tasks to be finished. If there 
are minor tasks with a fixed time length, they have been noted for your 
information, but do not need to be added to your response.
