Updated XML Encryption 1.1

I have updated XML Encryption 1.1 as follows:

1.  per ACTION-905 changed RSA 1.5 from Required to Optional, removed Optional/Required from algorithm description sections (in table of algs only). 

I also updated the Algorithm Cross-Reference accordingly.

2. per  ACTION-908 added new Appendix A for "Reserved Algorithm Identifiers" and moved AES-128/192/256-pad symmetric key wrap algorithm identifiers and description to this section. Removed those from table of algorithms section.

3. Fixed formatting of schema and examples , ACTION-904

I did *not* update the reference to SP800-56A to the newer draft reference as it is a normative reference and maybe we should only reference final material (ACTION-907). Perhaps this will be final by the time we go to REC and we should update then (comment?)
