Is the xenc11:KeyDerivationMethod element used only when 
the value of the Algorithm attribute is "http://www.w3.org/2009/xmlenc11#dh-es"
or "http://www.w3.org/2009/xmlenc11#ECDH-ES"?  In other words, when 
this attribute value is "http://www.w3.org/2001/04/xmlenc#dh" or
something else, is the xenc11:KeyDerivationMethod allowed?
