minor glitches wrt 'p:insert'

In its signature, the 'at-start' option is marked as required, but a
default is specified in the prose -- I think I like the default, and
so it shouldn't be marked as required.

In Example 5, the effect of the displayed insertion is described as
"An hr is inserted before the h:div and the resulting version replaces
the original h:div." -- this should read something like "An hr is
inserted as the first child of each h:div, and the resulting version
replaces the original h:div."
