We're proposing that at the end of a pipeline, the default output
falls out of the default output of the pipeline.  This isn't what you
want for other unconnected default outputs.  So there has to be a
difference.  I'd prefer the difference to be that "default output can
only be unconnected at the end of a pipeline" rather than that
"default output can always be unconnected, but is thrown away except
at the end of the pipeline".
