Updated editor's draft

I've updated the editor's draft at

  http://www.w3.org/XML/XProc/docs/langspec.html

to reflect decisions on the "low hanging fruit"[1] issues. I also
fixed the bug where we didn't specify that the default readable port
is, you know, the...default readable port :-) in p:input and friends.
