Maybe a couple of typos in XProc WG Note

I noticed a couple of things that may be typos.

Do you mean "steps to accomplish what the pipeline author".

http://www.w3.org/TR/xproc-template/#err.inline.C0026
> An example of p:document appears in Section 1, "Introduction".
I don't find any "p:document" there and wonder what is meant.
