XML Schema namespace document

Hi folks,

Persuant to our conversation of a few weeks ago, I've taken a stab at
drafting an updated namespace document for the XML Schema namespace:

http://www.w3.org/XML/2014/10/xmlschema-namespace.html

(I don't propose to publish the diff marked version, that's just for
our discussion.)

I didn't take the time to make the updated links absolute, but I will.
There's room for discussion however, about what, for example

  http://www.w3.org/2001/XMLSchema/XMLSchema.dtd

should return.
