URW3 final report check

Dear URW3 contributors,

Part of the publishing process is running the document through the  
W3C validator and link checker, and this resulted numerous changes to  
the HTML.  Most of these do not show up on the visible page but a few  
do.  Also, as can happen while you are "correcting" things, I may  
have introduced errors that change the display.

Therefore, I am asking once more for people to give a quick check,  
especially for sections with which you are familiar, so we can catch  
glitches before publication.  For use cases, this is both section 5  
and appendix A.
