ACTION-451: text on extensions

I've merged the text on extensions (ISSUE-133) in to section 4 (the
one on definitions), with some editorial changes to satisfy the gods
of good grammar.

I'd suggest that we don't close ISSUE-133 quite yet, since this text
mgiht (as discussed at the face-to-face) see some changes when I
rewrite the overall conformance model section.

Or, put differently, I think ISSUE-133 also blocks on ACTION-446.
