editorial: remnants of preconditions and effects

Hi Joel, Holger, 

I have identified a few remnant mentions of preconditions and effects in
the current editor's draft: 
- first example in section 2.2 
- the WSDL example in appendix B
- the schema in appendix B
- the OWL ontology in appendix D

Please remove them at your convenience. 8-)
