How do I create a new issue?

In my attempt to advise one of our members on how to create an issue in the
issue tracker (project.webplatform.org), I was almost completely flummoxed
in trying to create a new issue.

Wasn't there a "create issue" button previously? What happened to it?

Creating a new issue is not straight-forward, and the UI for doing so seems
to be all but missing.

How should community members create new issues?
