WebID hash questionnair feedback

Hi I put together the following questionnaire today using the w3C WBS tool.
It seems like one cannot view it yet, as I set the date for it to be open
for this Saturday after the teleconf. So below is the PDF

Currently the voting allows people to set preferences in their choices, and
to set some preferences.

Please let me know if you have problems with this.
