[SRI] format of the integrity attribute

In order to move the discussion forward in terms of the format of SRI
hashes, here's a concrete proposal for what the metadata might look like
if we move away from ni:/// URIs:

This proposal uses the same syntax as CSP Level 2 for the hash and
relies on the element's type attribute for the MIME type (which already
exists in all of the elements we plan on supporting in SRI v1).
