[w3c/manifest] editorial: link or remove unused definitions (PR #1251)

Fixes the four ReSpec "nothing links to it" warnings. The applied display mode now starts as the result of the chosen display mode steps, which nothing called before; the CSP section links to fetching a manifest image resource; uses of "solid fill" link to its definition; and "icon" is no longer a dfn, since nothing linked to it.

This change:

* Makes editorial changes (changes informative sections, or changes normative sections without changing behavior)
