Re: [w3c/push-api] Documentation: It's not clear how to specify the push service (Issue #355)

As far as I understand, 
- the push service is given as a constant by the user agent (browser) vendor. 
- the *Push Subscription* contains the endpoint URL, which needs to be forwarded to the application server:

I think, making these points clearer in the docs might help.
