[whatwg/url] Editorial: cleanup references (#299)

- Use abstract-op to refer to ToASCII and ToUnicode
- Use www.unicode.org domain consistently
- Make all terms defined in another specification visible in "Terms defined by reference" section
- List all dependent standards in "Infrastructure" chapter
