[whatwg/streams] Should ReadableStreamDefaultReader be exposed? (#586)

The spec doesn't seem to cover which classes should be exposed where.

Right now `ReadableStreamDefaultReader` isn't exposed on the global. Should it be? Feels like it should be exposed somewhere for extensibility.
