[w3c/DOM-Parsing] Apply serialization algorithm fixes based on testing (#14)

Numerous small bugs found when writing a polyfill for the XML serialization algorithm when comparing it to implementations. While the testing is not complete (is it ever?), now is a good time to sync up the spec to the various changes made to the algorithm in the polyfill in order to improve interop.
