[w3c/DOM-Parsing] Editorial refactoring (#10)

Prep work for additional normative changes to the algorithms

  * moved some sections around
  * removed external link processing by pulling external links into the dependencies section
  * Removed all usage of title= for linking
  * Switch to contiguous IDL
  * Moved sections so that APIs are defined ahead of implementation notes
  * Finish formatting and split-out the serializer algorithm into sections
