Re: [manifest] When created, navigate to start url (b9fde73)

I prefer to avoid SHOULD entirely (SHOULD is only useful in "RECOMMEND" form) - particularly here, as the point is to load the start URL when the application context comes into being. 

Please note that the start URL !== `start_url` member (the user or user agent may have changed it when the site was added to home-screen or otherwise bookmarked). 

Maybe I should add the above to the spec to clarify.
