I quickly compared it with Vangelis' form at 
<http://www.syros.aegean.gr/users/evlach/tcdlform/build/tcdl.html>.
Vangelis' form also asks for a date (but a date can also be generated 
at the back end, I suppose).
We also ask for a title (without specifying any requirements for the title).
With regard to technology, we also ask to specify the URL of the spec.
We don't have fields for HTTP headers or notes, although they look 
like a good idea.

Status information should definitely be available. (Filtering test 
cases on status would be useful.)
