I quickly compared it with Vangelis' form at <http://www.syros.aegean.gr/users/evlach/tcdlform/build/tcdl.html>. Vangelis' form also asks for a date (but a date can also be generated at the back end, I suppose). We also ask for a title (without specifying any requirements for the title). With regard to technology, we also ask to specify the URL of the spec.
