Web form interface for submitting test samples

Hi,

A while ago, and before we had the metadata in place, Chris Ridpath had 
created a draft form for submitting test samples (attached). I think it 
would be good to create a new one based on the latest metadata. This 
will be a first step to automate the structure review and to check any 
issues with the metadata (in terms of improvements that can be made to 
make it more friendly).

Any volunteers?
