TCDL draft D (with TCDL example)

Dear task force participants,

I have slightly updated the TCDL draft. The changes are the following:
- a description of xml:lang and hrefLang was added;
- a description of expertGuidance was added;
- the namespace and schemaLocatin were changed to avoid confusion with 
BenToWeb's internal namespace and TCDL schema;
- an appendix with a minimal TCDL example was added.

The example from the appendix is also attached as a separate file.
