Cleaned-up version of the EPR Scheme Note

Minor edits are displayed in the text.  Ginny's last set of comments are
addressed; however, I still feel the text is sufficiently clear on the
notion of what needs to be "hard-coded" in the model consumer to
dereference an EPR scheme.  It didn't make any changes in regard to that
issue.  Others may speak up.
