IMPORTANT: Combined Redesign Survey

Folks, 

Happy Monday!

Please take a few minutes to review & revise the Combined Survey for the various W3C audiences: http://www.w3.org/wiki/Headlights2013/SiteRedesign/CombinedSurvey.

The survey begins with universal questions that will determine the appropriate survey silo (public, member, chair). I've noted dependent questions (in green) and instructions & field types (in red), as well as a few questions re: the content of the questions themselves (bold). 

The survey seems to be taking shape but the chair portion is the least developed, so please weigh in.

We'd like to finalize the surveys by EOD Wednesday (1 May) to allow ample time to get them into the survey tool, test them and be ready to launch next Monday.
