Survey on changing meeting days and times

Note that the first question asks for a straw poll of overall day and 
time considerations. The second question asks for specific days and 
times.  This is deliberately a narrow band.  If enough people cannot 
make a time during that band, then we will use the first question to 
plan how to go forward with another survey.
