New version of RDF Terminology mapping

There is a new version of the RDF Terminology mappings:

Based on the last discussion I have
. Aligned it with the Primitive Datatype document
. Added Codeable Concept
. Added ValueSet declaration mapping
. Added ValueSet bindings

This is a first exploration in to the ValueSet area and will evolve. All comments are welcome.
