[remote-playback] Define behavior when "Establishing a connection with a remote playback device" fails

The algorithm defines the successful case but not the failure case; 
from the rest of the spec, it sounds like a `connecting` to 
`disconnected` state change + event should be fired at remote (versus 
`connected` for the successful outcome).
