This PR updates the spec to be more explicit about the in-sequence and
 reliability semantics of the messaging channel, and should close out 
Issue #46.

This does not seem controversial, so will merge right away.

Fixing the cross reference will be addressed later with a pull request
 to whatwg/xref.
