FLD/BLD cleanup

Over this weekend, Harold and I have made changes (both additions and clean
up) to the new FLD and BLD documents.

The main addition is the restoration of EBNF and XML to the BLD syntax
section, and the major effect of the cleanup was fixing the semantics of
equality and elimination of inconsistencies and omissions. We did many other
improvements, clarifications, etc., so the document should look better
now. Most of the changes are in the BLD doc.

Please use the Wiki version (or generate your own wiki-tr, if it works).

Reviewers: if you have already done a significant chunk of your review --
sorry for the changes.  Your comments on the old version are still very
valuable, but please check if the new version has addressed any of the
problems that you have undoubtedly discovered.
