Done. Text, schemas, XQuery, XSLT and examples updated.

the spec.

Changed wording of section 2.1 (Header): removed wording that linked
result binding order to header order.

Changed section 2.3.1: removed wording that linked result binding order
to header order, changed wording to reflect removal of <unbound/>
element from format. Included sentence to explain that unbound variables
are not represented by a binding element.
