I have moved the sentence ""A literal result element, or any other element
within a stylesheet module that is not in the XSLT namespace, may similarly
carry an xsl:use-when attribute." to remove this potential misunderstanding.
I have also added a note to the section on shadow attributes to clarify the
effect of attributes whose name begins with underscore on literal result
elements, extension instructions, and data elements.
