Notes:
- xsl:evaluate can be switched off or on, this is not listed in Appendix F, but
is, I think, also an implementation defined and allowed feature.
- xsl:evaluate may have a limited set of functions, not just extension
functions, like limiting use of fn:doc. This is not defined in Appendix F
