In the syntax summary we show xsl:accumulator/@initial-value as optional, but
the prose doesn't say what happens if it is omitted. We could say that the
default is an empty sequence - provided that is consistent with the @as
attribute - or we could say that it is mandatory. I think my preference is for
making it mandatory.
