Today I just noticed that Appendix C has been changed to indicate that it's
only talking about static+dynamic contexts for *modules*, which is incorrect:
it's talking about *all* static + dynamic contexts.

(A)
Section 2.1 refers to it for all contexts.

(B)
The column "Can be overwritten or augmented by expressions?" is clearly talking
about contexts for expressions.

(C)
If it were only about contexts for modules, there's no corresponding
appendix/section about contexts for expressions.


(The commit message for the changes says that it's integrating Ghislain's
proposal from
<https://lists.w3.org/Archives/Member/w3c-xsl-query/2012May/0118.html>, but he
didn't propose these changes to appendix C.)
