While checking the coverage and error coverage of the Full Text Test Suite, I
observed that there are not yet any tests for three errors: FOCH0002, XQST0013,
and XQST0079, even though all three of those errors are used in the text of the
spec. 

Descriptions should be added to Appendix D, Error Conditions, for those three
error codes.
