In 2.4.1 Insert (and possibly elsewhere) it is stated that the order of the
inserted nodes is preserved in the new document. This could be read as
referring either to the order of these nodes in the sequence that results from
evaluating SourceExpr, or to their position in document order. I suspect the
former is intended.
