DN-FO-12: 15.1.14 fn:subsequence -- examples

The examples are incomplete because $item4 and $item5 are not defined
(please, note that a reader may visit this section of the document directly
through the corresponding link in the TOC, being therefore unaware of the
source xml documents and variable bindings provided in a parent section).

Suggested solution:
	Make all examples self-contained, or add at the start of all examples a
link to the missing definitions.
