Standoff markup issue

Everyone,

Fredrik raised an interesting question during the test suite webinar and
that is the output for standoff markup for the test suite currently it is
like this:

The input files look like this:

Feedback on this topic would be nice and the output guidelines will be
updated on the slides based on this discussion along with the output where
applicable.
