LDP paging updated with June 30 resolutions

Ashok's email's comments are not addressed, just the June 30 resolutions.
There are a couple of items marked "todo" in HTML comments, so anyone 
reviewing might look at those too to be sure that they're clear on what 
(by default) won't be updated prior to the LC draft.  Of course, there are 
two other editors who might have time to help ;-)
For any further on-list comments, it would be useful to segregate them 
between those that Must be resolved prior to LC (you would -1 publishing 
LC if not resolved) and those more editorial in nature that could wait.
